JotForm

The Wildcat Club now uses JotForm to submit and approve all Check Requests, Wish Lists, Modified Wish Lists, and Honor Cord Orders. Please create an account using these directions so that you can track the status of these requests/processes.

WILDCAT CLUB MoDIFIED WISH LIST ONLINE FORM

If a team needs to make changes after the original Wish List has been approved, the team must submit a Modified Wish List. The Modified Wish List form is intended to replace the original Wish List, so all original items must be included - along with how these items have been revised (pricing, quantity, etc).  If you are needing to add NEW (never approved items) to your Wish List, please fill out a new Wish List form above.

WILDCAT CLUB HONOR CORD FORM

• Athletic Honor Cords are ordered in the Spring for each sport to present to their Senior Athletes for graduation. The cords are ordered by the Wildcat Club in one bulk order to achieve the best price. Prices are approximately $2.50 - $3.00 per cord. Please note by completing this form you agree to have the cost for the Athletic Honor Cords charged back to your team’s sports account.

TEXAS HOTEL STATE TAX EXEMPTION FORM

Please use this from to be exempt from paying STATE TAX at a Texas hotel.  Per Texas State law, Educational Entities are exempt from state hotel tax, but not local hotel tax. This exemption category includes in-state and out-of-state school districts, private or public elementary, middle and high schools, Texas Regional Education Service Centers and Texas institutions of higher education. A letter of tax exemption from the Comptroller of Public Accounts as an educational organization is not required, but an educational organization might have one. Out-of-state colleges and universities are not exempt.

COSTCO SALES TAX EXEMPTION CLAIM FORM

Please use this form to be reimbursed FROM COSTCO if you had to pay sales tax upon check out. After completing the form (you will fill out the highlighted portions), email it to lhwildcatclub@gmail.com for the necessary signature. Once it is signed electronically, you will receive a copy from The Wildcat Club. It is then UP TO YOU to submit the (1) form, (2) our Tax Exemption Certificate, AND (3) your receipt to apply for the exemption with Costco. The Wildcat Club doesn't guarantee that Costco will accept your exemption claim.
